Under supervision, you will perform a range of functions,ncovering multiple pharmacy locations, units or other settings. Your dutiesninclude the preparation and dispensing of drugs in all inpatient settings;nreview and verification of medication orders and patient medical records;noptimizing safe medication therapy; review of patient medication profiles;ncounseling patients on appropriate drug use; interacting and/or rounding withnother health professionals to help provide safe and effective use of medications;nprovision of advanced clinical services (e.g., pharmacokinetic dosing,ntherapeutic interchange, IV/PO conversions, TPN and aminoglycoside monitoring,npharmacist interventions, anticoagulation) to neonatal, pediatric and adult patients according tondepartmental policy and procedures, with possible focus on pharmacy specialtyn(i.e., oncology, pediatrics, geriatrics, ambulatory care); oversee dispensing,nreturn and waste of controlled substances on assignednshift/unit. Initiate, innovate and complete clinical, operational and/ornmanagement projects, in collaboration with Pharmacy Management. You will worknin assigned centralized and decentralized Pharmacy areas and patient carenunits.Â

Qualifications
We’renseeking a dedicated clinical professional with:

- n
- Pharm D. degree and current CAn Pharmacy licenseÂ
- ASHP-accredited residencyn (preferred)
- 2n years inpatient pharmacy experience (preferred)
- Exceptional communication,n interpersonal, organizational and follow-through skills.
- Knowledge of and ability ton perform age specific and disease specific pharmacokinetic calculations.
- Ability to compound intravenousn admixtures and other sterile products.
